Basic Term Life Insurance

Provides a monetary benefit to your beneficiary upon your death from any cause. The coverage is equal to one times your annual benefits base salary.

Accidental Death and Dismemberment Insurance

Provides you or your beneficiaries a monetary benefit when you suffer an accidental death, dismemberment, or loss of sight, speech, or hearing. Your benefit amount is equal to one times your annual benefits base salary.

Optional Life and AD&D Insurance

In addition to the basic life and AD&D insurance, you have the option to purchase additional life and AD&D insurance on yourself and your eligible dependents at a low rate.

Short-term Disability

Provides you with a weekly benefit payment if you are determined by a physician to be disabled. Payment will begin after 30 consecutive days of disability and may continue for up to 26 weeks. You are eligible to participate in this benefit on the first of the month following 90 days of
full-time employment, at no cost to you. You will also have the option of increasing the benefit another 20% at a minimal charge to you.

Long-term Disability

Provides you with a monthly benefit payment if you are determined to be disabled by a physician. Your monthly benefit will begin after 210 days of consecutive disability. You will be enrolled in this benefit on the first of the month following 2 years of full-time employment, at no cost to you. You will also have the option of increasing the benefit another 20% at a minimal charge to you.
